How come that a moderator was able to get into an Administrator only area?

could you provide some specifics. Was a moderator able to view the admin panel? Could the moderator have been assigned both mod and admin roles?

By the sounds of things it could be very possible that this moderator has been assigned permissions via another usergroup. So even if you restricted this to admins. If the person is in a usergroup it's very possible they could see the forum.

When I assign Moderators, they come up on the Admin Widget. Is this a bug, or is it meant to be?

Hi, truth is half and half ;p
What I mean by that is it was designed to show both, but it used to clearly state which were admins and which were moderators.
It now longer does this, which is a bug.
So in conclusion, it is doing what it should, but its not being as clear as it should.
